7Accident of Yeti Airlines DHC-6 Twin Otter:
9N-AFE at SURKHET Airfield on 29 June 2007

1. Injuries to Persons
There were no injuries to the crew or to any persons outside the aircraft.
2. Damage to the Aircraft
The aircraft suffered extensive damage to its airframe around the nose gear. The nose oleo strut was completely broken off at the base of the strut and lthe nose gear with its fork was lying off twenty feet to the right side (north side) of the aircraft.
The right wingtip had scraped both the tarmac and the grass outside of the runway as the scraped both the tarmac and the grass outside of the runway as the aircraft veered to the left. The wingtip was completely damaged and a large tuft of grass and clay was still hanging onto the wingtip at the time of inspection.
The right main landing gear was half buried in the soft clay. The right tire was completely flat and a piece of the right tire hub had broken off. One propeller blade on the right engine was seen to be slightly bent.
3. Probable Causes
The First Officer was the Pilot Flying (PF) and had conducted the landing. However, after touchdown, during the landing roll, when the aircraft began veering off to the left, the Captain was unable to utilize all means necessary, to effectively bring the aircraft back to the centre line. This is the probable cause for the incident.
4. Recommendations
Bpilos are required to undergo a Refresher Class

Captain Bijendra Shrestha is required to fly 25 hours with Instructor Pilots and a further 50 hours with Line Captains before being released to fly with copilots.

First Officer Roshan KC is required to fly 50 hours with Instructor Pilots before being cleared to fly with Line Captains.

The Company is strongly advised to take caution when rostering Junior Captains with Junior Co-Pilots in the future, especially to and from STOL fields.
- Non-IP rated Captains are to be cautioned against providing "flying instructions" to junior pilots.

Operations Director and Flight Safety Director to regularly monitor and evaluate Junior Captains as well as overall crew performance.
